Can I level a concrete pad myself?

I have a patio in two sections each one is 10' X13' one section has settled due to poor drainage. My question is, can I relevel the pad myself by undermining the low side and lifting it with car jacks and then pouring a concrete footer for extra support.the shift is approx. 1 to 1.5" at the seam. I have fixed the drainage problem already.
